弹性成像技术联合血清学模型评估乙型肝炎患者肝纤维化分期

摘要

Objective: To observe the value of real-time tissue elastography (RTE), shear wave elastography (SWE) and serological models in staging of liver fibrosis in chronic hepatitis B (CHB) patients. Methods: A total of 112 patients with CHB and glutamic-pyruvic transaminase (GPT) less than or equal to 2 times upper limits of normal were enrolled. The parameters of RTE, SWE and serological results were obtained, including shear wave velocity (Vs), liver fibrosis index (LF Index and F Index), GPT, glutamic-oxaloacetic transaminase (GOT), γ-glutamyltransferase (GGT), platelet (PLT) and so on. Taken results of histopathological examinations as golden standards , the receiver operating characteristic (ROC) curves were drawn, the areas under the curve (AUC) were calculated, and the diagnostic efficacy of each index and the GOT/PLT ratio and GGT/PLT ratio of serological models in diagnosis of liver fibrosis stage were compared. Results: AUC of Vs, LF Index and F Index for diagnosis of liver fibrosis (≥F2 stage) was 0.952, 0.889 and 0.938, respectively, of F4 stage was 0.930, 0.858 and 0.918, respectively, of the combination for diagnosis of liver fibrosis ≥F2 and F4 stage was 0.975 and 0.951, respectively. Combining the above 3 elastography indexes with the selected serological models the GOT/PLT ratio and GGT/PLT ratio, AUC for diagnosing ≥F2 and F4 stage was 0.979 and 0.954, respectively. Conclusion: Combination of RTE, SWE and serological models could improve the diagnostic efficiency of staging liver fibrosis in CHB patients with slightly elevated GPT.
